These flexible assessment, treatment & intervention services are currently delivered to young people who are entitled to statutory throughcare and aftercare services and who are also experiencing difficulties through their own substance misuse.
These services provide:
- Personalised support packages designed to meet individual needs.
- Specialist substance misuse screening and assessment during Pathway Plan development and at reception into accommodation.
- Support to parents, family members and carers through informal consultation, information and training as required.
- Negotiation of appropriate medical and clinical supervision according to the needs of the individual.
- Participation in case conferences, children’s hearings, court proceedings, childcare reviews giving information to assist the decision making process
- Liaison with housing professionals regarding the difficulties looked after young people may experience in obtaining and sustaining suitable tenancies.
- A leading role in establishing effective multi-agency working practices between all agencies and services and contribute to the co-ordination of service delivery.
